What this document is

One door into a fragmented system

The most extensive document in the set — an executive framework that integrates Veteran Directed Care (VDC), VA Community Care, Louisiana Medicaid HCBS 1915(c) waivers, and PACE with LCDC's CAPE™ model into a single statewide navigation system.
